9600 Pro Odd Problem

I think my card is startin to die. It cannot take stock speed anymore so i have to underclock it quite a bit. The fan is fine and i put a new coat of thermal goo on it to make sure. Uses a Vantec Iceberq 4 HSF. I even put a fan in the case that will blow air across both the bottom and top of the card. So I've ruled out heat. But this may have something to do with the flash mod I tried to do with it. I tried to mod it into the FireGL card using the BIOS flash guide over at Rojak's Pot and it seemed to work. Then I modded it back to the original BIOS. Thats when it started texture tearing and then reseting about every 10-15 mins while playing a game with VPU Recover. It stops doing that if I underclock the core by 50+ MHz. Is there a BIOS utility where I can erase the BIOS completely and add the original back instead of it being overwritten or is this just a case of the card dying. I've had the card for almost a year now. I'll post specs of my comp if needed. Thanks.
 
What drivers are you using? I tried the same mod you talked about, didn't work for me off the bat, so I flashed right back, had no issues. But I did have similair problems with cat's above 4.6, i couldn't even get past the menu in BF:V without vpu recover popping up. Went back to the 4.6's and my problems were solved, I tried underclocking to fix it too, after going back to the old drivers I could even OC again.
 
Voltage is fine. Its actually above what its supposed to be so. I'm using the 4.9s. I never had this problem with the 4.7s or 4.8s though. I'll try going back to the 4.6s and see if that helps. To specs...

EDIT: Ok, I tweaked the voltage a bit and also went back to the 4.6 cats. Still no effect. I underclocked it to 303 Core to see if it has any effect. Haven't tested that far yet though.
 
Well, card is still doing the same thing. The video will shut off and then come back on. I guess my card is just dying. What are the good cards under $200 that are as fast or faster than the 9600 Pro? I wouldn't mind a 5700 Ultra or similar.
