4850 - Display driver has stopped responding

I just recently built a new pc with a 4850 and have had this problem happening: screen goes black, recovers and I get the message 'Display driver stopped responding and has recovered - Display driver atikmdag stopped responding and has successfully recovered'

My hardware:
HIS 4850 Iceq4 Turbo
E8400
DFI Lanparty JR P45 T2RS
Corsair 2x2gb TWIN2X4096-6400C4DHX
Corsair HX520 PSU

Things I've tried:
1) Catalyst 8.10, 8.11 and beta 8.12, with & without ccc. Wiped drivers between each install
2) Tried 32 and 64 bit versions of Vista (was using 64 initially)
3) Downclocking from 650/1000
4) Removing one vista hotfix that supposedly helped someone with this issue on some forum
5) On the hardware side I've run prime95 and memtest86+ with many many cycles and no errors.

This issue happens all the time, and there doesn't appear to be any specific trigger. I could be opening an image in a viewer, watching a video, playing a game, etc.

Does anyone have any other suggestions on things I could try or could this just be faulty hardware?
 
Let me guess, you have dual monitors, probably running at different resolutions?
Lots of other people have been reporting this problem with Vista and dual monitors. The fix seems to be to either disable Aero and use Vista Basic, or use only one monitor.

I was getting the exact same errors every day, but since switching to Vista Basic a week ago, it hasn't happened since.

I guess 8.12 hasn't fixed this issue either.
 
Nope, single monitor. Benq E2200HD. Everything running at stock too. No aero either.
 
Swap out the card and find out if its a hardware issue. It could also be bad RAM, unstable CPU, any number of different things. I've seen bad RAM pass 6+ hours of memtest, so I would also suggest just trying a single stick at a time in different slots. Don't assume that a pass in a stress test means its stable. ;)

I would also try the video card in another system and see if it works.
 
Yeah, I was getting the Atikmdag errors for the longest along with other erros... Turned out to be a bad stick of RAM, even though 80% of the posts with people trying to trouble shoot the problem blamed it on faulty drivers, windows, etc...
 
Guess I'll check the RAM a stick at a time. Not related but when I tried to run memtest86 overnight last night, the dvd drive wouldn't read any cds, just dvds. I swear this build is going to kill me.

My friend has been having similar troubles - took ~4 months to determine that his 9600GT woes were due to bad RAM (after an RMA'd motherboard didn't fix anything). Oddly enough, his DVD drive is also messed up now, although it thinks it is a floppy drive for some reason, lol.
 
i get the error every now and then but it only happends if im folding and trying to watch tv on my pc with my tuner card.

 
Thanks for all the replies. It's down to either RAM or mobo. Pulled out one stick yesterday and haven't seen the black screen since. Will be running memtest extensively on each stick individually to confirm this as soon as I get my dvd drive working again.
