I cannot find these PCI IDs in the mapping files of ESXi 5.1. Are you sure that this controller worked with ESXi 5.1 before? Or were you just passing it through to a VM?
If it worked before then I guess it used the generic ahci driver. But this can of course only work if...

- When adding tgz files you might need to choose the option "Force repacking" with some packages (this will prevent a "Boot image corrupt" message when trying to install later). This includes Chilly's e1001e.tgz file!

This shouldn't be related. The HA (or FDM) agent is deployed to the hosts by vCenter when you enable HA/FDM on the cluster. So, it is not part of the install ISO.
