I can reproduce it with every catalyst release, and a lot of users confirmed it in their 6xxx and 7xxx cards too.
I reported it at least three times using their contact forms (12.8, 12.10, 12.11)...
Can you provide more info (i.e. your Catalyst driver / Windows version / your browser name / hardware acceleration enabled or not / a screen with your temps / potential errors in event log)?
I discovered that this bug is caused by atioglxx.dll related to 11.8/11.9 driver versions. A workaround is to place atioglxx.dll from Catalyst 11.7 into the same folder of the program with an opengl issue (caused by 11.8 or 11.9 drivers).
So this bug is part of "OpenGL Extension Limit" issues...
An exception is thrown when an wglCreateContext is called when OPENGL32.DLL is dynamically loaded (using dynamic DLL linking). AIDA64 corrected this issue in latest beta using static linking of OPENGL32.DLL. Probably we should send this thread to AMD, because the reference in OP doesn't clarify...
Wonderful, I just note that AMD fixed many bugs in this version (not noted in official release notes).

1) Existent bugs to update:
wglCreateContext Exception = NOT FIXED in 11.9 (as stated here)
DCOM Error = FIXED in 11.9, as stated in release notes.
I haven't verified other bugs in the OP, because I don't use CCC. Any other help would...
DCOM error seems fixed in 11.10 preview, but I'm waiting to try 11.9 final drivers for a definitive sentence.

Approximately you could increase manually UVD/3d voltages to 1.163 with RBE, maintaining 850/1200 on both...but in this case you should also uninstall catalyst drivers before to flash a card. After flashing your card, re-install catalyst drivers to apply new clocks.

The last post in that topic suggests to underclock the memory to 1150 if you've experienced any previous problem with original 3d clocks.
But reading your post, your problem is only the UVD mode with 400/900 clock speed.

Reading some topics in this section and in other forums, it seems to me the UVD priority bug is not limited exclusively for HD5000 series. Or, at least, this is not the only reason.
IMHO drivers behavior can still improve a lot, beginning from DCOM issue (apparently involved with 2d/3d clockspeed).

Sorry but the 'UVD information' spam in event viewer is not minimally connected with the 'DCOM error' spam in event viewer. In my previous post I reported two different bugs.
