This might help you a bit... If you ctrl+left mouse click on the layer it will select the border of the image in that layer. Thus, if you have 5 small circles on one layer and you ctrl+left click on the layer, the 5 small circles will be selected.

I don't know what Photoshop is exactly doing but if you use a mean or median filter, I think you could achieve something similar. Using one of these filters on the resulting image would reduce the noise level. This page briefly describes how the filters work...
