I have two suggestions:
First in audio properties highlight your playback device and click propeties. Then go to enhancements tab and select voulme equalization.
Second download an install AC3 Filter. I know your opposed to packs but this is only one program.
Depends on your definition of large. I had a 4850 in mine with room to spare. For most, I think the spacing of the slots n the motherboard is going to be the limiting factor. Replaced it with a passively cooled card only for noise reasons.
Standard ATX12 Power Supply, full ATX motherboard, full height expansion card slots, room for more fans then you'll ever need (120mm and 80mm) and easy to work inside of. Only real cons is that it's kinda deep, could be a problem for some entertainment center and the LCD info panel is sold...

Range is great, mouse and keyboard can both be used one handed and it's inexpensive. Some complain it's not backlit but it hasn't been a problem for me.
Unfortunately you can't easily force it not to disable audio. One aspect of Digital Rights Managemant (DRM) is the the HDMI Protected Audio Path (PAP) which forces a reset of the audio when it detects something is added or removed. They are ways around it but they center on defeating the DRM...

I had the same problem. For me it was something in the settings but I can't remember what. I want to say it was either a video or audio item. Whatever it was, once I unchecked it it worked like a charm. Try playing around in there.
On second thought, a new tuner would be the best solution. The PVR500 only support NTSC, newer ones have ATSC (Over the Air Digital) and Unencrypted QAM (Digital cable - what is and isn't encrypted varies by cable company). At apx $100, the HVR 2250 is hard to beat with it's dual hybrid tuners...

Make sure Jumbo Frames is set to 5K or 7K, Realtek disables them by default. No promises but it made a night and day differance streaming from my Windows Home Server to my HTPC.
