I own a Zalman Card Reader which enables me to plug in various memory cards such as SD, M2, Micro SD, SIM etc. It connects to the USB header on my motherboard and I use the "Realtek 090901 All in One Card Reader" drivers that I got from the Zalman website. In device manager, the device is shown as a "Realtek USB 2.0 Card Reader"

Sometimes when I am using the my PC (Windows 7 x64), the card reader is not being detected by Windows and appears as an "Unknown USB device". Reinstalling the drivers does nothing, uninstalling the device, rebooting and installing the device does nothing. Even powering off the system does nothing, the card reader will not work. To get the card reader detected by Windows, I have to power off my PC, switch the off button on my PSU, wait for the card reader light to turn off, and then turn on my PC. When I do this, only then does the card reader work fine. It will work fine for days until I have to perform the above again.

Why is this?

*picks a card out of the deck* Because of the gravitational pull of Jupiter :) . Seriously, It could be any of a whole host of issues, but I would say it is just failing and time to get a new one. You could try moving it to another (if available) USB header to see if it is the port that is failing and not the reader.

The card reader is new, just a few months ago. I will try another header next time I give my PC a clean inside. :(
