Windows File Accessed Times

What operations actually change the accessed times of files in Windows? I've been updating tags and covers in my FLAC files and noticed that changing the information in tags only changes the modified time, but adding new cover art also changes the accessed time. But if I update the cover art a second time, the accessed time doesn't change.

Why is this?
