You're not doing anything wrong. My sapphire 4850 doesn't OC wroth crap either.
 
What is the temp difference in your case compared to the temp in the room?

75c is horrid. I would never try to overclock something that was running that hot at load. 60-62c is the most I like to see when something is overclocked.

I bet you can lower that temp by a good 5-10c or more just be removing the side off of the computer case... the 4850, blows all the hot air back into the case... makes everything run hotter.
 
4800 series have the bios set to run at low fan speeds. So many people are unaware of this and freak out when they read about the temps.
