RAID Controller/Drive Troubleshooting

I recently purchased an LSI MegaRAID SAS 9271-4i and 4x WD Red 4TB drives. I'm attempting to create a RAID 10. I've went through the process of creating the array but instantly after the process is finished, the array degrades with the drive connected to port 2 dropping out. To troubleshoot the issue, I switched the SATA cables for the drives in port 2 & 3 and recreated the array from scratch. Once again, I get an error with the array degrading with drive on port 2 dropping out. Also, I updated the firmware for the card during all this to eliminate that.

So my thinking is the RAID controller is bad or the cable is bad. Due to this being my first experience with a RAID card, I want your opinions on this before I ship the card back.
 
I would start off replacing a cable. I had a cable go back recently that had some VERY strange issues go with that bad cable which made me thing it was a bad HDD, until I had problems with a different drive on the same cable.
 
I ended up getting a replacement card and everything is running fine now.

My next issue is heat. The chip temp is running 100C+ so I know I need better cooling. Unfortunately, air flow with the fan config I have is not going to be adequate for the card. Instead of installing higher CFM case fans, I would rather cool the card with a local, small fan. I would like to slap a 40mm fan on the heat sink but I need a way to attach it. A Noctua NF-A4x10 would be perfect. Any recommendations? I would also be open to a replacement heat sink/fan combo is such a thing exists for this card.
