1) What will you be doing with this PC? Gaming? Photoshop? Web browsing? etc
PC Gaming (Modern Warfare 2, Left 4 Dead 2, and a few other games) and other typical computer uses.

2) What's your budget? Are tax and shipping included?
$1100 plus tax and shipping.

3) Where do you live?
Brooklyn, NY. Zip Code: 11208

4) What exact parts do you need for that budget? CPU, RAM, case, etc. "Everything" is not a valid answer. Please list out all the parts you'll need.
CPU, Monitor, MOBO, RAM, HDD, GPU, PSU, optional DVD drive (don't see real use)

5) If reusing any parts, what parts will you be reusing? Please be especially specific about the power supply. List make and model.
Lian Li PC-P60b Case.

6) Will you be overclocking?
In a couple of months after build is complete and I have money to buy a cpu cooler (don't need a good cpu cooler now, stock is fine).

7) What size monitor do you have and/or plan to have?
Need a monitor as well. Preferably above 23".

8) When do you plan on building/buying the PC?
Less than two weeks from when this was posted.

9) What features do you need in a motherboard? RAID? Firewire? Crossfire or SLI support? etc.
Don't really have any special features in mind.

10) Do you already have a legit and reusable/transferable OS key/license? If yes, what OS? 32bit or 64bit?
Yes, got the win7 pro upgrade :-D I think its only 32 bit though.

This is what I have in mind. Would like you guys to critique and add in any offers with deals attached.

Some of the things below include shipping and taxes, while some don't which is ok. I think I can go $30-$40 over budget without too much trouble.

Motherboard - EVGA x58 Micro $200
Video Card - EVGA GTX 460 1gb $246
Power Supply- OCZ ModXstream 600w $70
RAM - Corsair Dominator 3gb $118
Monitor - Dell sp2309w $209 (after dell coupon)
HDD - WD 750gb $60 (after newegg coupon)
CPU - Intel Core i7 930 $200 (microcenter)

Thanks for the help guys.

3 GB more for just $17 more

GIGABYTE GA-X58A-UD3R LGA 1366 Intel X58 SATA 6Gb/s USB 3.0 ATX Intel Motherboard $210

cooling on the evga x58 micro isn't the best. Gigabyte x58a is the best performance board in that price range.

XFX P1-650X-CAG9 650W ATX12V 2.2 / ESP12V 2.91 SLI Ready CrossFire Ready 80 PLUS BRONZE Certified Modular Active PFC Power Supply $100

OCZ Modstream are horrible PSU, its better spend little extra for good psu like the XFX.

I have read about the problems with the evga x58 micro NB cooling, however I already received a thermalright NB heatsink that a friend gave for free. That should clear up NB heating issues.
