Monitor Problems

Humm. I'm having monitor issues. @60 Hz, there are two slow moving dark "lines" that move from the top of the screen to the bottom and cycle like that. It's not hugely noticeable, but it's there nonetheless (it's more noticeable with different backgrounds etc.). At anything above 60 Hz, I can notice quick horizontal lines that flicker. Does anyone have any idea what's going on?

I'm running two 6800 GT's from BFG. My monitor has a VGA cable that is connected to DVI adapter and then connected to one of the graphics cards. Now, each card has 2 slots for a total of 4 monitor slots. Only one of them is working right now. How do I get the others to work? (in case the issue is with that particular slot, I'd like to be able to test things out)
