Hooking LCD to Xbox...

You just plug the yellow cable into the input. You'll of course need to give it 12V.
As for headphones, get yourself one of these and one of these, or something that perhaps does both. I didn't see anything that would on radioshack.com but you may be able to find something that will at a different place.
 
How would I get it 12V? AC/DC converter? Battery?

To get 12V from your XBox - use the yellow (+) and either black (-) wires that are attached to the hard drive.

I have to get inside the xbox to power the screen? Is there an easier way?

How would I put a AC/DC adapter on the screen, so it could be separate from the xbox? Where do I get the adapter?

That seems pretty damn easy to me... But if you want it to be external - you just need an ac/dc adapter that outputs 12V. Any electronics store should have something suitable. Otherwise - you could order one from Jameco, Mouser, or Digi-Key (or any of the other millions of electronics stores out there)
 
Just one more plug and wire. It really isn't that hard. Pop the xbox open andcut the insulation of the wire without cutting the wire. Easiest way in my opinion is to use a soldering iron with a flat tip. Then simply attach your wires to the yellow wire (12+) and a black wire.

Now if you are not mounting this lcd inside the xbox than you can use something like this:

The xbox end being the jack and the panel end being hardwired in.
