Help needed troubleshooting DXDIAG errors

Been experiencing crashes and lockups recently when gaming, both in Mass Effect 3 (even when using the Catalyst Crossfire Mass Effect 3 profile) and in BF3 (when using the BF3 Crossfire profile). The fans on the GPUs will often start spinning really really fast and really really loudly, but there's not always a correlation between the fan ramping up and these errors. Not sure if it's particuarly AMD or Crossfire related issue. Anyone got any ideas what might have caused this error message?

Wondering if I should ditch Crossfire and buy a single new NVIDIA 680 card.
 
I've never seen though errors before (even on my CfX setups).

Those errors scream hardware failure of some sort to me though.
 
Have you tried running with a single card to see the issue still occurs?

Try running dxwebsetup.

Was this after a driver update (possibly rollback)? Or some other update?

Clean install?
