DP Active adapter broken?

So my third monitor on my 6870 is no longer waking from sleep / monitor shut off / system reboots

When I unplug the DP adapter and plug it back in to the next DP, sometimes it reconnects my third monitor, other times CCC pops up an error message about the adapter not being appropriate or something.

Just wondering if this happens and if its usually the DP adapter rather than the card.

edit: definitely not a software problem because it also affects the BIOS and boot sequence
