1) What will you be doing with this PC? Gaming? Photoshop? Web browsing? etc
DX10/11 Gaming, Writing, Web browsing, Photoshop and Ilustrator use. Probably in that order.
2) What's your budget? Are tax and shipping included?
Under $1100 - 1500 if humanly possible
3) Where do you live?
WI
4) What exact parts do you need for that budget? CPU, RAM, case, etc.
OS, CPU, GPU, RAM, PSU, MoBo, RAM, Optical drives, Storage and Boot drives. Cooling, and cables. Possibly a case, though I have an empty one I may use at least to start out.
5) If reusing any parts, what parts will you be reusing? Please be especially specific about the power supply. List make and model.

Preferably, depending on bang for buck available
7) What size monitor do you have and/or plan to have?
3x 20" 1680x1050
8) When do you plan on building/buying the PC?
Completed by end of February '11
9) What features do you need in a motherboard? RAID? Firewire? Crossfire or SLI support? USB 3.0? SATA 6Gb/s? etc.
Crossfire, unless Nvidia has a comparable solution that I'm not aware of, Raid isn't necessary, but plentiful, fast SATA connections are.
10) Do you already have a legit and reusable/transferable OS key/license? If yes, what OS? Is it 32bit or 64bit?
Need to purchase 64bit Windows 7
I'd like to do as well as I did with my last build if possible. It's lasted me since April 2005 with only a CPU/GPU upgrade in 2007 and pretty constant additions of storage, now pushing the 5tb mark.
As far as what I think I need, I'd like a single card GPU solution that I can add a second card to when they're cheap. Since I'm running 3 monitors, this pretty much limits me to ATI/AMD for the GPU. Fan noise does not bother me do I'll be cooling with air.
As I noted above, I do have an empty Cooler Master 690 series I may be building this in, though I am open to snagging a new case IF, it has a minimum of 6, easy access, 3.5" bays. That is the only thing I don't like about my current Stacker 810, it's been the perfect case otherwise.
Current Solid State drives are probably out due to the fact that my Steam installed folder is pushing .5Tb most of the time.
Deadline for completion of this build is Feb 28th. I could extend into March only if something is going to drastically alter the Price/Technology land scape before then.
