P2V: vCenter standalone converter fails

trying to convert a 2003 server using version 4.0. it fails out right after you specify destination saying "fatal error occured. the most common reason is loss of network connection." no firewall on the box being converted, nor the one doing the conversion. boxes have good connectivity.

according to the log its having an issue with the disks, but my google-fu has not come up with any info to help solve this. any ideas?

the last couple lines of the log:

looks like the agent cannot get the disk information. what is the disk config on the machine being P2V'd? what controller, how are they connected, is there an active or passive backplane in the loop, what size, kind of drives are they, etc?
 
got any antivirus installed, or cisco VPN?

dynamic disks, any iscsi/nfs targets on there?
 
antivirus is off, no cisco vpn. comp management shows 3 partitions all basic/ntfs. hardware manager shows a perc 4e/Di card and two adaptec 39160/3960D ultra 160 cards. no idea on physical configuration for disks/raid setup. i can't power the box down for a closer look till sunday. :/
 
could be the volume management on one of the RAID cards is causing the agent to behave erratically. or, maybe one of the controllers has no disk attached, and the there's a timeout on the bus scan occurring.
 
I beleive withj 2003 is uses VSS to snapshot the volumes. Do you see any VSS error in the event logs ?
