    Replace one of your (2) SAS breakout cables (pic in 1st post) with a SATA breakout cable; and get 2x "SATA-to-SAS_HDD" adapters like [Link] for the 2x "odd men out" 4TB SAS HDDs. How did you miss seeing "22-pin SATA to SFF-8482 adapter" in all your searching?

That indicates cables had been right/good all along; also that there had been no power supply anomalies in your tower/DAS, which could have explained the symptom, I can not envision that merely bad/wrong firmware in your original cards could explain it...

    Superficially, they will look the same; the difference is the pinouts/wire-connections. The description on that product page makes it clear that it IS a reverse breakout cable. If a 50cm long cable will suffice, This is cheaper. Disclaimer: I am not 100% certain that [SAS] reverse-breakout works...
