    It sounds like a driver issue, the card keeps locking up. Uninstall all the amd drivers, boot into safe mode and run driver sweeper : http://www.guru3d.com/category/driversweeper/ make sure everything is removed and use the 12.2 drivers that naninani posted when you boot back into normal...

    Cheapest I know if you have the adapters : http://www.newegg.com/Product/Product.aspx?Item=N82E16814150574 It's $30 per adapter though for mini-dp. Otherwise adding a cheap pci-x card in addition will also work.

    I've never been able to get all 5 outputs to work at the same time, it seems one DVI and HDMI is shared on my card (XFX 6970 2gb). I finally gave up on this after reading AMD' spec which says : "Native support for up to 4 simultaneous displays."...
