As obvious as it sounds, check your driver stuff to make sure its not doing weird stuff with profiles or something (my geforce stuff had a tendency to do that at one point, i dont know if AMD software is similar)
im sure you've tried all that already-cant hurt to mention though.
I cant imagine it would be anything but a bad card. Although if possible i would try to test the PSU with a similar power draw in another PC or with a different card, in case something was terribly wrong with it, but that seems highly, highly unlikely.
