I also had this on my Supermicro server board, I tried the EFI tool instead of the DOS version - the flash failed for me using UEFI boot method (built into the Supermicro BIOS).
I swapped the card into another board and used the standard DOS tool method. They EFI tool may of worked but after...
I managed to recover the card - not sure 100% what the real issue was or what actually revived the card. Many reboots and failed flash attempts and erases with little result.
The LSI flash utility is much better than the Dell version (much later version so to be expected) - I used the P14...
