My original card was likely faulty or flashed incorrectly. I received a refund and purchased a new, unopened 9201-16e and everything works properly on the same hardware.
For any future searchers, try an RMA if you can't flash a card pulled from a Dell server especially if you don't have access...
Turns out they had a Poweredge 2850 that doesn't have PCI-e so I'll probably just RMA the card. In your experience Rifter0876 if the 9201 doesn't come from a dell server is it usually motherboard agnostic?
