OK this list is pretty weak. But I have to give props to the Bubble Bobble reference. As a kid, when he came on stage, I was up out of my seat trying to quickly finish the stage. So I can KIND of see why he's there :)

What would you put down that just made you really uncomfortable when they were on screen or just heard them?

Also most of the creatures from the silent hill series, they always come out with disturbing stuff. The best ones are the babies from the first series that you can't kill, they sure freaked the fuck out of me every time they popped out.

The concept of the flood is pretty scary, your endless hordes of mutated freaks coming to eat you.
 
I don't know what the name was, but on our old - old - mac there was this isometric game where you piloted a spaceship or something over a load of lava. I think it was hell-themed. Anyway, when the time ran out there was this sort of shriek and this massive floating head started chasing you about shit-your-pants fast.

That's the scariest enemy I'v eever found in a game. If anyone could name the game for me, that would be ace. It's bugging me.

Where are the mind controlling mutants from STALKER ?
Where are the invisible ninjas and especially Alma from FEAR ?
 
These types of lists almost always come out completely bass ackwards.

I think scary enemies are largely so because of the setting they're in, for pure terror I'd have to go with Alma from FEAR in both the freaky little girl form and the disgusting, rotting, teenage psudo-real life form.

Props go to some of the mutant things in the underground levels in STALKER, the insane mental patients of the Shalebridge Cradel in Thief 3, the aliens in AvP singleplayer (as the human ofc) and probably some of the later enemies in Doom 3
 
One that always scared the pants off me was Sinistar (old arcade game), when you heard him yell 'I LIVE' and 'Run Coward!!!!!' you really did.
 
My dad, playing video pinball on Atari 2600. He would taunt me and kick my ass. (wow, that's been nearly 30 years ago!) We still fire that game up on occasion. I need to play this weekend.
 
Oh also...I remember me and 2 mates playing the shareware wolfenstein demo, I think it was just the first chapter of the full game. We got to the end room which was obviously a boss, so we got right up to the door, and one of my mates (who wasn't playing) said there's probably a large guy behind there with a big mini gun.

And the guy who was playing was like "Mini guns, mini guns, thats all you ever go on about, would you just shut up about it", promptly opens the door and of course "GUTEN TAG" and here stands a massive dude with a minigun pointed right in your face.

So I scream in a kind of half-scared half-messin' way, and of course my mate screams and it panics my other mate whos playing, so he jumps and sort of loses the ability to control the character which ends up in this dance of trying to close the door to lock him back in, while the boss is trying open it...it was just screams of panic for a good 20-30 seconds which eventually broke into hysterics and had us laughing so hard we couldn't breathe.

Looking back on it, not that scary...but we were kids and it was funny at the time :)

No joke. Playing stalker through for the second time makes it more scary, rather than less. I got to that part pictured last night, and knowing what was coming the dread just built up as I waited for him to make an appearance. I completely shat my pants when he made the charge.

How were any Half-Life enemies anything more than a nuisance?

Those bastards just dont die. First time I encountered him I barely had any ammo to begin with. Damn thing pissed me off.

Invisible ninjas in FEAR were annoying as hell and I had a lot of cussing moments with them as well. First time I encountered them in that office I had no idea what was killing me until I started shooting around like a madman and damaged one.
 
"The Suffering" had some scary characters, especially the guys who had long blades for arms and legs. I actually had a few dreams of being chased by those bastards.

those shadow babies in Silent Hill. *shudder* We used to play that game on a Playstation with all the lights turned off. I couldn't play for long. I'd chicken out. And then I'd have dreams of that darn baby laugh. *chills*
 
System Shock 2 hybrids and midwives are pretty disturbing/scary.

Yeah the suffering had some creepy characters, i loved that game

A few years ago, I picked up a platinum edition DVD of Max Payne 1 and 2. I never played them, but this Jack Lupino character sure makes me wanna dust off that DVD and install it.
