XP Pro Start button problems

my friend asked me to take a look at his computer. for some reason his start button lags 10-15 sec when you press it. I disabled most startup items and tried to run the OS with minimal startup options using MSCONFIG with no success. Ran adware, spybot search and destroy, antivirus with latest pathces and still the problem presists. Right now im thinking it might be a 3rd party program causing it, but i dont want to go ahead and uninstall program after program.

Do you think it might be a problems with windows itself? Has anyone else seen this type of problem?
 
right click my computer --> properties --> advanced --> performance settings --> visual effects = best performance

then go to display properties --> appearance --> effects --> take off ALL CHECK MARKS
