The game was originally much more GTA like with tons of activities to do like fighting in a fight club for money, betting on the race track, drag strip racing, etc. There was also a respect/reputation feature so if you were a classy gangster or were well respected, people would respect you more and offer you lower prices or lower bribes, etc. and if you were a cold blooded killer who harmed the public (police, civilians, etc.) your reputation would go down and police would go harder after you.
Look at the full list of the original save points. There were other missions set in WWII before the one in the final game! There are far more missions and even the 4 different branches to the 4 endings. There's even a minimap of the Italian campaign on the wiki.

Jimmy's Betrayal/Vendetta was originally going to be a single large DLC called Rags to Riches but Sony gave 2K a bag of money and they got the exclusive rights to half the story and so they just rushed Jimmy's Vendetta to market with scraps from the leftover cut missions (the mansion) without cutscenes or any storyline and broke it in half and made it a Sony exclusive.
There was also a big job system with tons of jobs you can do which also includes choices your character can make.

There's also a ton of public transport cut of the game. The Taxi system is mostly complete. The subway was modeled and there are even tunnels running through the 3D map. There were trains, buses too, etc.
You know all those neon signs showing clothing/diners/gunshops, etc.? Well there were also signs for buying houses from the realty shop and also for buying cars from Diamond Motors (that's why the interior is fully modeled).
