request for design - barcodes

hey guys,

can i bother one of you to help me out with a little something

i need a barcode (like the ones that are scanned, or to be more specific, like the one on the back of agent 47's head for example) that reads the following

15 01 1983

would be great if you can help me out, im planning on getting this inked under my arm, kind of like a 'meat tag' thing.

what type of barcode? There are many different formats... UPC,EAN, 3of9, etc.

You can find 3of9 or UPC fonts on the web and just install them into windows.
