Little boxes appearing around stuff on the screen?

My mom is having a problem with her iMac with OS X...there are little boxes appearing around stuff on the screen such as text, buttons, etc. Apparently there is only 1 box at one time on the screen - maybe based on what button I click, what window is open, something like that? I have no idea how it happened, and I'm not sure if it's a feature (for disabilities or something like that?) or a bug or something.

Has anyone heard of this? I'm not a Mac expert at all but I seem to be obligated to help her since I know a lot about computers. Any advice would be appreciated - thanks!

System Preferences > Universal Access > Turn Voiceover OFF

Uncheck "Enable access for assistitive Devices"
