exchange/outlook issue? emails randomly going to deleted and junk folders

I have a client, she gets emails from the same people every day. She is bat shit crazy so it might just be her but she says that emails from say [email protected] will sometimes go to her inbox, sometimes to her junk email and sometimes to her deleted. All emails are of business nature. Nothing spammy about them from what I can tell.

Have you guys ever heard of this
 
Check rules in Outlook
Add senders to Safe Senders list
Delete .OST file
Check client into mental institution or teach how to use a computer
