Error with pfsense and hard drive

I've gotten this error several times now and what i found on google and pfsense are not working for me. I have tried 3 different hard drives and keep getting the same errors.

Pfsense locks up and quit handing out DHCP addresses is how i notice their is a problem. I attempt to reboot it but can not. So i hard reset it. Does a fdisk check and then sits at this screen.

Anyone have any ideas on how to fix this?
 
Most likely that is caused by a failing hard drive. The error messages posted indicate its having trouble reading a set of LBA addresses on the current disk (ad4). Back up your PFSense configuration, check cabling and run the drive manufacture's diagnostic tool on it, and be prepared to RMA or replace the drive.

This happen on 3 different hard drives that i have tried. Forgot to mention this in the first post.

Hardware as follows

Atom D510 with 2GB stick of ram
Two Intel Gbit NICS ( On mobo)

SATA hard drive
 
What kind of SATA controller is this drive attached to? Have you also tried changing the SATA cable(s) and ports the drive is attached to?
 
I changed the controller mode in bios and also the cable and the port it was plugged into. Crashed again... :(

The controller is Intel ICH9R SATA 3.0Gbps Controller on the mobo

Is there some pattern to when it crashes? How did you install pfsense on this system?

If the hard drive tests out okay then we need to look at other factors such as the power supply or mainboard.

Yes i do have pfsense on this system. I installed from cd, version 2.0.1.

Well i powered up the box today to see if it would error upon boot up. It booted up but looking at log it seem unable to mount / with read/write access.

Also complaining that filesystem is dirty.

Have you run fsck?

You can't install pfSense on a partition, it wants the whole disk. If you can't devote an entire hdd, it's perfectly happy on a thumb drive.
Is /dev/ad6s1a a slice you created?

I have not but fsck after reinstalling pfsense. (since creating this thread)

I think i partitioned it at one time because i was receiving DMA errors. So results on google were saying to use a smaller drives. I thought i try just making a partition smaller in size to see if that would help. It didn't.

I don't recall if it was this reinstall or another one. Any way to check what the partition are?
 
Well looks like i have multiple partition. They are showing unused. am i reading this output correctly? (see below)

Well i recieve the same errors as before now.

Ran fine for awhile then i shut it down to re-rack it and booted it backup. Worked for about an hour then started throwing errors about hard drive was unreachable and timed out. Couldn't write xxxx, expact data loss.

no inodes free.


I think the controller has had it :(
