its hard to tell from that picture, but it seems to be a card issue... your card could be overheating, is you card overclocked? is there a lot of dust in the heatsink? is the fan running (at full speed or at all)?
it could be dying from other reasons, but check those things and see if it fixes it
if you have some AS5(or somthing similar) take the heatsink off and clean off all the old thermal compound with rubbing alcohol and put some fresh on it...it could simply be that the card is dying from "old age"... electronics dont last forever, so it may be time for a newer card
clearly its not the monitor if you're taking a screen shot of it, and see the problem in the screen shot...its deffinately your video card. Try different drivers and see if that helps. If not, I think you're gonna need a new video card.
