I have an Antec 1200 V3 which you may know comes with three of the 3-bay internal hard drive cages. Is it possible to pick up a fourth one of these for the top slot? Will it fit? I do not need 5.25" bays and would prefer to have an even stack of 12 3.5" HDs for a storage pool.

You could find some 5 in 3 adapters and get 20 drives in there...
