As an onsite tech that works with spyware/malware every day I can honestly say that Spybot in an integral part of my tool kit and I wont consider a machine clean without running it.

I use AVG free and a combination of the server 2k3 firewall and peerguardian2/protowall as needed. I also run Spybot/adaware/hijack this about every 2 weeks or as needed.
