So far my replacment card is working fine. No crashes. Running GPU-z shows an idle temp of 55 and 46 degrees Celsius. Not sure why there are two temps listed. Are these normal temps? fan is at 60%.

I was able to keep the same install even with an entirely different board. Windows just made me re-activate my license. If you've done this too many times you might have to call in to get a verification code.

I was running on the 191.07 drivers since they came out and most games ran fine, including DA. I did notice some artifacts on floor boards after I updated to the 195.xx drivers though. This didn't last too long though as my card died the next day. :D
Would removing the heat sink void your warranty? I wanted to do this to my 8800 but there is a portion of the sticker that covers some of the screws. Unless I am just thinking about the plastic cover on the fan and not the entire heat sink.
Today my missing card was received by BFG despite my forgetting to put the RMA form inside the package. Hopefully all goes well and my next card will not have faults.
Ok, so after a few days of panic, my package made it to the RMA center and has been received. Yay! Hopefully my next card will last longer than 5 min of use. I hope the next card has better luck. I was thinking of using it as a PhysX card.
Just a thought. Have you tried taking the card out and air dusting the crap out of it? I once found a huge dust ball rolling around in there that I had to use a Q-tip to get it out.

Sounds like what happened to my GTX 8800 OC2. RMA'd it and a replacement is on its way. If you don't want a dual card slot you should RMA it anyway, if you find that it is bad. Then sell the replacement to offset the cost of a new single slot card. Have you tried a driver cleaner yet? I'll post...
