Windowed full-screen whenever possible. If that isn't an option, I go fullscreen.
 
Exclusive full-screen, always. I have a PG278Q using G-sync, so it's a requirement. But even before then I ran all my games the same way. I don't need to be distracted by other things while playing a game.
 
I do full screen. I have a second monitor so anything else I want up just stays on that.

Plus.like others I'm running a PG278Q so... Gsync.
 
To me, it completely depends on the game. MMO's I will sometimes play in windowed mode. Almost any other game, full screen or nothing.
 
I like windowed fullscreen mode. It's really a must with multiple monitors. So many games don't support that for whatever reason -_-

For Skyrim I had to get a "fake" windowed fullscreen mode program but it worked well (Fake Skyrim at the Skyrim Nexus). Still, wish all games supported it without mods/third-party programs.

Borderless window is a feature in the Windows Presentation Foundation library, which replaced the old GDI/DirectDraw way of creating windows in the Windows SDK when Vista was released. It is not a requirement for Windows applications, so it is up to the developer if they want to use this feature in their games. Unfortunately shortly after SP1 was released for W7 Microsoft decided to bring back GDI, DirectDraw, and Direct2D due to demands from developers, so I have a feeling that the movement forward in application development for Windows is slowing to a crawl. The good news is that developers seem to have taken notice in the demands for "windowed fullscreen" and we're now seeing more games being released with this feature.
