All the time. All ages (2 days to 6 years later) and all brands of media, both CD and DVD.

A few weeks ago I was using name-brand media to put files on DVD. Last week 2/3rds of the files had a CRC failure.

All my important stuff I keep on RAID 5 hard drive arrays.

I've seen some people ghost their home pc. Personally I don't see the point of it because a failed HDD hardly ever happens to me that when I do restore my ghost image the programs I have installed are simply very old versions and I have to try to fetch them again. I just backup the original exes and burn them on dvd once it gets to that size. If I ever get a hard drive failure I just do a complete format (since it would be a new HDD this is where I'd have to start).

Work pc backup is more understandable, but home pc is just a wait of time and energy.

I buid computer systems for a living from home and I have seen many Hard Disk drives fail.
I have piles of them sitting in boxes.
Recently a 4 month old 120Gb Sumsung SATA, 30Gb Samsung, many Maxtor drives dying or going dead via the spindle bearing "Whine of Death".
For a while in the past I had 40GB WD's dropping like flies.
Optical drives tend to go almost as much as hard drives.
I beleive dust is what kills optical drives off and, beleive it or not, lack of use.

1. Have you ever had a hard drive fail on you? Yes

2. Have you ever had a USB Pen fail on you? (if you've used these) Sort of, I have one WindowsXP box that will not recognize any flash memory devices but those same devices work just fine in another computer. I believe the WinXP USB mass storage driver is hosed an I need to remove it and let WinXP fix itself. Other than that, I do know two people that have had sandisk cruzers fail.

3. Have you ever had an external Hard Drive Fail on you? (if you've used these) N/A

4. Have you ever had a CD-R, DVD-R, or other optical media fail/corrupt on you? Yes, many

5. Have you ever deleted a file by accident and had it be unrecoverable? Yes

6. Do you have any sort of backup regimen? Not really :(

1. Have you ever had a hard drive fail on you?
Yes, two, but only when over 2 years old and both progressive failures. Never tried RMA etc

2. Have you ever had a USB Pen fail on you? (if you've used these)
No, but only been using one for 6 months or so (fairly heavily)

6. Do you have any sort of backup regimen?
Backup of important documents to second machine / CD, but no regular or complete backups

1. Have you ever had a hard drive fail on you?
Several

Yes

2. Have you ever had a USB Pen fail on you? (if you've used these)
No

3. Have you ever had an external Hard Drive Fail on you? (if you've used these)
No

4. Have you ever had a CD-R, DVD-R, or other optical media fail/corrupt on you?
No

5. Have you ever deleted a file by accident and had it be unrecoverable?
Yes

6. Do you have any sort of backup regimen?
Yes (sort of. CD)

1. Have you ever had a hard drive fail on you?

Many

2. Have you ever had a USB Pen fail on you? (if you've used these)

Once when I plugged it in to a miss-wired USB Plug (5v to ground=bad)

3. Have you ever had an external Hard Drive Fail on you? (if you've used these)

Yes and no. I put an old laptop drive in a case once and it died fairly quickly, but not because it was external, the drive was scavenged from a broken laptop.

4. Have you ever had a CD-R, DVD-R, or other optical media fail/corrupt on you?

many times

5. Have you ever deleted a file by accident and had it be unrecoverable?

many times

6. Do you have any sort of backup regimen?

To a point, I upgrade drives once in a while and currently have my old drive still sitting around. So, in some ways my "backup" comes from laziness of not doing something with the old drive.

1. Have you ever had a hard drive fail on you?
-Yes, I have had multiple HDDs fail on me
2. Have you ever had a USB Pen fail on you? (if you've used these)
No
3. Have you ever had an external Hard Drive Fail on you? (if you've used these)
No, I have not.
4. Have you ever had a CD-R, DVD-R, or other optical media fail/corrupt on you?
Yes, I have had discs go bad.
5. Have you ever deleted a file by accident and had it be unrecoverable?
Not if I realized my mistake in the same session. But Yes it has happened.
6. Do you have any sort of backup regimen?
Yes, Images, backup hard drives etc..I like to go through backup steps once every 2-3 weeks.
CD/DVD backup becomes useless when you are talkng about several hundred gigs of video, audio and the likes. With HDD so cheap I have actually built a machine for the simple use of having HDD space that is less likely to crash (Due to almost never being used) so I can keep all these files in 2 places. Its a bit of a waste, but losing that info would be pretty crappy. The new Hi-capacity DVDs should be fun to use though.

Cheers

6. Do you have any sort of backup regimen? Alas...no.

I have never had a drive fail other than from physical damage caused by me (broken IDE pin, collapsed cover). I have always used Western Digital drives.
