What case design do you want to see made?

How about a traditional layout with 44mm cooler height for the Thermalright AXP-100 without a fan with an ATX PSU right on top as a CPU cooler as well? SilverStone had cases with cooling like that, I actually have an SG02/ w the NT06-E doing exactly this. That'd be 130mm which is what you need for a video card anyways. The fan in an ATX PSU could easily provide CPU cooling. There's an empty space on the front right, two 3.5" bays being 50.8mm this would easily fit without interfering with the PSU cabling with the 147 long angle along the motherboard. That'd mean at least 170mm+102mm=272mm long video cards are supported which is necessary anyways because higher end cards are 10.5". Still only 10L (calculated with 155 height, 290 length, 222 width) and would be cheap to create and populate, no expensive SFX PSUs, no risers. Basically a cheaper Sugo SG05 revised for a 10.5" GPU and ATX PSU but with slightly lower height.

Have you looked at the SG13? It's basically what you're describing. Taller by an inch, but it only supports 61mm tall CPU coolers in 181mm of height, so in order to fit 44mm tall coolers yours would still need to be around 165mm tall.

Hrm that could work, the AXP-200 is 60mm high after all. That's a little higher than I hoped for, however the AXP-200 instead of the AXP-100 is nothing to scoff at. It's interesting how the case is made with the ATX PSU up or down both Silverstone Sugo SG13 Mini-ITX Case Review Photo Gallery - TechSpot http://www.inter-asia.com.my/wp-content/uploads/2015/05/SG13-4.jpg with the latter picture having well visible screw holes for the other directions.

I am really wondering whether I'd need the mesh front case, the ATX PSU fan really ought to be enough, aside from the K CPUs nothing is above 65W these days.

A single fan, positive pressure filtered design. I hate dust and fan failures.

Similar in concept to a Mac Pro.

Perhaps using one big 140mm intake fan and flowing through a bunch of passive heat sinks and out the opposite end.

Well phanteks have that new PSU adapter that makes it possible to use one PSU with two systems - Phanteks Innovative Computer Hardware Design

dunno how that works if you power one of the systems down though.

Hm, that would by quite big, you'd probably need to make something like it and bundle it with the case, but it can be done. I think the only thing this PCB does is split the power cables to multiple outputs. Maybe it includes some protection circuitry for the PS_ON signal, but other than that there shouldn't be anything on there. Even if its just multiple splitters in a box, you should be able to power each system down separately. A motherboard doesn't care whether the PSU its connected to is putting out power or not, it only turns on softly (by power button, wake-on-USB, power restore action, stuff like that).

I guess something like this in a smaller formfactor would work perfectly fine for the kind of case you're thinking about.
 
I'm trying to design a slim case and don't want to open a thread just for this, can anyone tell me what's the standard distance between the metal top of the CPU and the bottom of the mobo (including its PCB)?

Unfortunately, there is no standard for this. From the top of the PCB to the top of the CPUs IHS (integrated heat spreader), it's specified to be 7.542, I think. The PCB is normaly 1.65mm thick, I believe. ATX specifies there has to be 6.5mm below the PCB for components, but if you look into the standard it should turn out that 4.5mm work just as well (don't take my word on this, look into the standard yourself!).
