Pulled from a working server, fan is making some noise, but still spins.
*note the fan leads are soldered (from the factory) onto the PCB..cut/splice is an option I guess

This card is model PEXG6i, looks exactly like the one in the link, except this one has a small (40mm? fan) on it.
I installed it in my Win7 64bit desktop and it had built in drivers, shows as "Intel Pro/1000MT dual port adaptor" (hah, 6 of them)
Looking for $60 shipped
