cd rom duplication and packaging? any referrals?

I have a client looking to have cd's made up to be used as mailers to advertise his business both via handouts in his office and via mail. I am trying to find somewhere online that will do both duplication (pressed cds) with artwork on the cd and then the packaging (preferrably slim jewel cases) and then the insert.

He's looking for around 100 to start with. Ive googled 'cd duplication' and found some sites, but I'm just looking to see if anyone on here has done something like this before and can refer me to a site you went through and had a positive experience with. thanks!

Here are a few places to start with. Keep in mind that the higher the quantity of the run the cheaper the price. For example, it might be the same cost to replicate 500 as opposed to duplicate 100, depending on the terms of the job.
